Inhaled Corticosteroids and Systemic Effects in Children

Inhaled corticosteroids (MSG) has been known among doctors as an effective therapy to cope with chronic respiratory diseases, such as asthma, cystic fibrosis and allergic rhinitis. In children, KSI is considered safer than oral corticosteroids (oral KS) for systemic side effects are not as many oral KS, where the use of oral KS frequently reported occurrence of growth disorders, diabetes mellitus, and renal impairment.

Along with the development of tools and formulations KSI, local side effects wane. On the other hand, the properties contained in oral KS it appears also in KSI. Several cases were reported and showed the cause was the use of MSG in the long term.
When this has provided a variety of dosage KSI, call metered-dose inhalers, dry powder inhalers, and nebulizers. Metered-dose inhaler is a device MSG is most often used to control asthma attacks. This tool is created with the goal of reducing corticosteroid deposition in the mouth and increase distribution to the lungs. With the reduction of deposits of corticosteroids in the mouth, the side effects of local case of thrush, pharyngitis, and cough can be minimized.

The goal of increasing distribution to the lungs is that the drugs work better. However, it also increases the amount of corticosteroid deposit in the lungs. This deposit is the main source of systemic absorption that would avoid systemic side effects, either HPA axis suppression, diabetes mellitus, and growth disorders. But this can be fixed by making formulations that enhance bonding with protein drug deposit so little is absorbed into the systemic.

Until today MSG is safe and effective, but the likelihood of systemic side effects remain. So that MSG should not be granted for a period of time and the doctor should carefully pay attention to when necessary to reduce the dose gradually in accordance with the guideline. In addition, it has been made on such additional checks, cortisol levels, blood glucose, and bone mineral density, in order to facilitate doctors to detect the possibility of side effects from the use of MSG. This recommendation is mainly done when to begin drug treatment, started raising the dose, or if the patient shows symptoms of systemic side effects after administration of MSG.
